Self Management Education
We can help you learn to take charge of your mental and physical health. Trained facilitators use a holistic approach to passing on knowledge and skills. They can help you to learn about weight loss management, stress reduction, diabetes self-management and to live more mindfully.
Our qualified facilitators help you learn Self Management Skills
Our Self Management Education classes encourage you to be proactive and take charge of improving your health and wellbeing. You can learn to improve your management of diabetes, weight reduction, living smoke-free and dealing with stress by using Mindfulness Based Stress Reduction. Several of our Self Management Courses are run in association with East Health Trust.
While you are taking a greater part in managing your health, we are here to support you by providing you with more information and options to plan the type of health care you need in the future. This may include setting out an Advance Care Plan.
